Tackling Gaps & Changing Processes for Success

Making the Case: How to Motivate Clinicians to Get You the Documentation Details You Need

Documentation is the key to ICD-10 code abstraction. Kickoff your ICD-10 Summit experience by going inside a clinician’s mindset to understand the burden they feel and the reasons for their resistance. Dr. Joe Nichols will show you how to best communicate to your clinicians the incentives and value most meaningful to them, in their language, that will urge them to produce high-quality, fully detailed documentation. After this revealing session, documentation resistance will no longer be a struggle at your practice!

  • 8:30 a.m. – 10:00 a.m.

    Decoding Terminology Traps

Now that you are aligned with your clinicians on the importance of detailed documentation, turn your attention to the documentation problems guaranteed to trigger audits. Learn how to avoid documentation short comings and terminology risk points to ensure your practice stays away from additional development requests and audits. Hint: if you’re seeing terms like recurrent vs. chronic or use vs. abuse vs. dependence used interchangeably in your clinician’s documentation, this session is a MUST.

  • 10:00 a.m. – 10:15 a.m.

  • 10:15 a.m. – 11:15 a.m.

    ICD-10 Documentation Audit Game Plan

Practices that were proactive and organized fared the best during the ICD-10 transition. Proactive planning and organization will reign again in the battle against documentation errors. The key now is to have a defined ICD-10 chart auditing plan. In this session, you’ll define and build the ICD-10 chart auditing plan that works best for your practice. Learn how to assess your risk, implement proven sampling techniques, spot inaccurate coding due to documentation deficits, routinely monitor improvement, and focus on individual re-training. You’ll walk away with an actionable game plan ready to implement at your practice when you return.

  • 11:15 a.m. – 12:15 p.m.

Documentation Gaps: You Hold the Power to Solve

Addressing and preventing documentation gaps requires out-of-the-box thinking. For some, the key will be modifications to existing forms and templates. Others might find system prompts they deploy in their EHR will be the answer. Or perhaps rearranging your intake process to collect better data before the office visit begins will be the winning solution. Join Dr. Nichols as he narrows down the best strategies and approaches you can take to work smarter, not harder, to address documentation gaps.

  • I was extremely pleased. As a Practice Manager, not a coder, I was less prepared than many at the conference. I now feel much more confident in my understanding of ICD 10 and how our entire practice will need to work going forward.
