Only then can you design targeted interventions to carefully and proactively manage care and results. While the concept may be easy to grasp, executing risk stratification isn’t so straightforward. Which method for stratifying a population by risk best aligns with your patient mix and goals (HCCs, ACGs, ERA, etc.)? Should you factor comorbidities into risk stratification; and if so how? How do you step into risk stratification as you build up better data? These challenges and more will be addressed in this session.
