The Case In Point Platinum Awards Program, like no other awards program in the industry, brings professionals and organizations together to highlight their work, be publicly recognized, celebrate their successes, and collectively recognize that working together as a team can ensure a safe, quality and sustainable healthcare system.
The Awards program recognizes both individual and organizational performance in a range of categories from the emergency department, to the medical home, to health IT and specialty programs, as well as leaders in case management and organizations across the continuum who are improving the delivery of care. Processes such as transition of care, disease management, behavioral health and other creative strategies are making a difference as the industry works to improve quality of care, while at the same time lowering healthcare costs. A new category has been added this year to recognize the important role health IT is playing in assisting professionals to collect and analyze data in order to identify gaps and improve outcomes.
Today, all eyes are on the practice of case management and care coordination as a means to improve the efficiency and effectiveness of patient care and outcomes across the healthcare spectrum. The Case In Point Platinum Awards provides a public platform to recognize the work being done in these areas. These efforts are decreasing fragmentation, ensuring safe transitions of care and improving the patient experience all while containing escalating healthcare costs across the care continuum.
This ceremony will recognize the finalists and announce the winners in each category. The Platinum Awards is part of the Care Coordination Summit that will take place May 4-6, 2015 at the same location.

Our keynote speaker this year is Dr. Mary Beth Bigley, currently the Director for the Division of Nursing and Public Health in the Bureau of Health Workforce at HRSA. Prior to that, Dr. Bigley was the Director of the Division of Science and Communications at the HHS’s Office of the U.S. Surgeon General. She received a doctorate in Health System and Policy at The George Washington University School of Public Health and Health Systems and currently holds an academic position at The George Washington University in the School of Nursing.
